Clinton Terry Schoonover is a 55 y.o. male who presents for follow up on pain in both feet and chronic neck pain s/p remote surgery. He has been seen by Oly Ortho spine for the neck pain and it appears they ordered a MRI or if not possible a CT myelogram (due to possible metal artifact). He would like a referral to pain mgmt because Pregabalin isn't enough, he wants opioid therapy which we don't do here for chronic pain. He needs to know the diagnoses of his feet for his attorney for disability. I told him what is in his chart- plantar fascial fibromatosis of both feet. DX codes Chronic neck pain with history of cervical spinal surgery M54.2, G89.28, Z98.890 Plantar fascial fibromatosis of both feet M72.2
